These are websites which provide wizards to easily make online forms: "Put an online contact form on your site in about 8 minutes. No programming required. (Not even a little bit)"
Also here are links to some information and tools for making your own forms.

Why are they free? How do they make money?: They may have ads.; They may offer a fuller service for a cost.; They may use it as an introduction to their company, or to other services which cost.; Some may be doing it to spread viruses or spam. So be careful.

Bravenet.com http://www.bravenet.com/ Very professional-looking, different kinds of forms and many other things for website. Also has a great web references section. Don't know yet if it has any kind of quota or limit for the forms and tools - looks like you don't find out until you sign up. Bravenet : 2009
